On July 12, 2023, Premier Health Partners discovered a significant data breach that affected its hospital and health care operations in Dayton, Ohio. According to the company, an unauthorized party gained access to its systems between June 7, 2023, and July 12, 2023. During this period, sensitive information belonging to patients and possibly employees was exposed.
The breach compromised a wide range of personally identifiable information (PII) and protected health information (PHI). The data exposed included Social Security numbers, dates of birth, digital signatures, driver’s license numbers or state-issued identification numbers, financial account information, health insurance details, individual taxpayer identification numbers, login credentials, medical information, names, and passport numbers.
The exposure of both PII and PHI increases the risk of identity theft and medical fraud for those affected.
Premier Health Partners has provided a detailed notice of information privacy incident on its own website, outlining the nature and scope of the breach. The cybersecurity incident was disclosed to the Massachusetts Attorney General's office on Aug. 13, 2025.
In response to the breach, Premier Health Partners initiated a thorough investigation to determine the extent of the unauthorized access and the types of information involved. The company has notified affected individuals in writing, as required by law, and has provided resources to help them understand what happened and what steps they can take to protect themselves.
Given the severity of the breach and the types of information exposed, individuals are encouraged to remain vigilant for signs of identity theft or fraud. It is advisable to monitor financial accounts, review credit reports, and consider placing fraud alerts or credit freezes with major credit bureaus. If login credentials were involved, changing passwords and enabling multi-factor authentication where possible is strongly recommended.
More information about the organization can be found on the Premier Health website.
A breach notice means your personal details could be circulating far beyond the organization involved. One practical step is continuous monitoring: services such as Identity Defender (included with an ExpressVPN subscription) can automatically check dark-web markets, flag new credit-file activity, and request removal of your information from data-broker sites.
This kind of “early-warning system” can’t undo a breach, but it can help you spot misuse quickly and limit further exposure. ExpressVPN is offering 61% off, risk-free for 30 days, with ID Theft Insurance included and no extra cost for those who sign up for one or two years.
