On Nov. 7, 2024, PhyNet Dermatology, which provides administrative support to Premier Dermatology Partners, detected suspicious activity involving an employee’s email account. Immediate action was taken to secure the email environment, but further investigation revealed that a limited number of email accounts had been compromised. As a result, sensitive information contained within those accounts may have been viewed or taken by unauthorized individuals.

The investigation, which required a time-consuming review of the impacted files, was completed on June 6, 2025. It was determined that patient information specific to Premier Dermatology Partners was involved in the incident.

The breach exposed a range of personally identifiable information (PII) and protected health information (PHI): full names, addresses, Social Security numbers, financial account information, dates of birth, medical history information, treatment information, diagnosis information, treating physician, medical record numbers, and health insurance information.

Premier Dermatology Partners's response

After discovering the breach, Premier Dermatology Partners, with support from PhyNet, secured their systems and launched a thorough investigation to determine the scope and impact of the incident. In response, they reviewed and enhanced their existing administrative and technical security controls and provided additional security training to staff to help prevent similar incidents in the future.

To assist those potentially affected, the company has set up a dedicated call center at 877-250-2771, available Monday through Friday, 9 a.m. to 9 p.m. ET, excluding holidays. Individuals can also contact PhyNet in writing at 302 Innovation Drive #400, Franklin, TN 37067.

Premier Dermatology Partners encourages all potentially impacted individuals to remain vigilant against incidents of identity theft and fraud. Recommended steps include reviewing account statements, monitoring explanation of benefits, and checking free credit reports for suspicious activity or errors.
