On Feb. 6, 2025, Family Centers Inc., a nonprofit civic and social services organization based in Greenwich, Conn., discovered that an unauthorized actor had potentially accessed personal and protected health information belonging to individuals associated with the organization. The suspicious activity was first detected on Jan. 30, 2025.
Information compromised in the data breach includes names, driver's license numbers, Social Security numbers, dates of birth, health insurance information and medical information. Family Centers disclosed the data breach to the U.S. Department of Health and Human Services on March 31, 2025.
The organization also published a Notice of Data Security Incident on its website. The total number of impacted individuals has not been released.
In addition to required state and federal disclosures, FCI has notified affected individuals. Family Centers set up a toll-free call center to answer questions about the data breach at 855-932-6459, Monday through Friday between 9 a.m. and 4:30 p.m. EST.
