
Individuals who received a notification from WebTPA Employer Services LLC or its co-defendants that their private information may have been compromised in a data breach may qualify to submit a claim for up to $5,000 from a class action settlement.
WebTPA Employer Services LLC, along with Hartford Life and Accident Insurance Co., Anthem Blue Cross Life and Health Insurance Co., and Elevance Health Inc., agreed to pay $13,750,000 to settle a class action lawsuit. The lawsuit alleges these companies failed to adequately protect private information, resulting in a data security incident that potentially exposed sensitive data of individuals whose information WebTPA and its business partners stored or processed.
Who can file a claim for a data breach payout?
Class members must meet the following criteria:
- They are an individual in the United States who received a notice from WebTPA Employer Services LLC, Hartford Life and Accident Insurance Co., Anthem Blue Cross Life and Health Insurance Co., or Elevance Health Inc. stating a data incident may have compromised their private information.
California subclass:
- They are an individual who resided in California between April 18, 2023 and April 23, 2023, and received the data breach notice. These class members may be eligible for additional compensation.
How much is the data breach class action settlement payment?
Class members can submit for one or more of the following benefits:
- Cash Payment A – Documented losses: Class members may claim up to $5,000 for documented losses related to the data incident.
- Cash Payment B – Flat cash payment: Class members who do not submit a documented losses claim may submit for a flat cash payment estimated at $100. The settlement administrator will determine the final payment amount by the number of claims filed.
- California statutory payment: Class members residing in California between April 18, 2023 and April 23, 2023, may submit a claim for an additional $50.
- Medical monitoring: All class members can elect to receive two years of CyEx’s Medical Shield medical monitoring, which includes medical identity monitoring, real-time alerts and up to $1,000,000 in insurance for medical identity theft.
How to claim a class action rebate
To receive a settlement payment, class members can:
- File a claim online
- Download and complete the PDF claim form to mail to the settlement administrator

Class members must submit or postmark the claim form the Nov. 4, 2025 claim deadline.
Required claim information and proof
- To submit a claim online or by mail, class members must provide the class member ID from the settlement notice.
- Documented loss claims require supporting documentation, which may include receipts, invoices, credit card or bank statements showing fraudulent transactions and other proof showing identity theft.

$13.75 million data breach settlement fund
The $13,750,000 settlement fund will include:
- Settlement administration costs: To be determined
- Attorneys' fees: Up to $4,582,875
- Attorneys' costs: To be presented to the court for approval at a later date
- Service awards to class representatives: Up to $1,500 each
- Medical monitoring costs: Determined by the number of claims filed and estimated at $90 per claim
- Cash payments to approved claimants: Remaining settlement funds

When is the WebTPA class action settlement payout date?
The settlement administrator will issue payments to approved claimants no later than 105 days after the court grants final approval of the settlement.
