If you received a notice from Reventics, LLC regarding a December 2022 data security incident, you may be eligible to submit a claim for up to $5,000 (or a flat $100) from a class action settlement.

Reventics, LLC and OMH Healthedge Holdings, Inc., doing business as Omega Healthcare, have agreed to pay $8.15 million to resolve a class action lawsuit. The lawsuit was filed over allegations that the companies failed to adequately protect personal and health information, leading to a data breach discovered in December 2022.

Who is eligible for a data breach settlement payout?

You are a class member if:

  • Your protected health information (PHI) and/or personally identifiable information (PII) was stored on the platform accessed during the December 2022 Data Security Incident.
  • You received a notice in February 2023 indicating you were an impacted individual.

How much is the class action settlement payment?

Class members can submit for one of:

  • Reimbursement for documented losses: Class members may submit to receive reimbursement for up to $5,000 in out-of-pocket expenses and monetary losses traceable to the data breach. This includes time spent dealing with issues caused by the breach, costs such as credit monitoring and professional fees, and other financial losses.
  • Flat cash payment: Class members that do not submit a claim for documented losses may submit to receive a flat cash payment of $100.

All payments are subject to a pro rata adjustment, based upon the number of valid claims filed.

How to claim a Reventics Data Settlement payment

To receive a payment, class members must submit a claim form by the July 25, 2025 deadline. You can file a claim online or download and print the PDF claim form to complete and mail to the settlement administrator.

Required proof and/or documenation

  • CPT ID and Passcode from settlement notice required to submit a claim.
  • Documented losses claims require supporting documentation such as receipts, invoices, police report, bank or credit card statements showing unreimbursed or fraudulent charges, and any other proof related to remedying monetary losses due to the breach.

When is the Reventics data breach settlement payout date?

Payments will issued to class members with valid claims approximately 60 days after the court grants final approval of the settlement.

Why did this class action lawsuit and settlement happen?

This class action lawsuit was filed after Reventics discovered a data breach on December 15, 2022, in which a cybercriminal accessed and encrypted information on its network. An investigation confirmed that private information had been acquired.

Plaintiffs alleged that Reventics failed to adequately protect sensitive data. The defendants these allegations, but agreed to settle in order to avoid the expense of continued litigation.
