Outcomes One Inc. agreed to pay $1.7 million to resolve a class action lawsuit alleging a July 1, 2025, data breach allowed a criminal third party to gain unauthorized access to an employee’s email account. The breach potentially exposed the names, demographic details, medical provider information, health insurance data and medication information of affected individuals.
Who can file a claim?
Individuals may file a claim if:
- They are a living individual residing in the United States.
- They received a notice from Outcomes One Inc. indicating the July 1, 2025, data incident may have impacted their private information.
How much can class members receive?
Class members can claim one of two types of cash payments plus an additional benefit:
- Cash Payment A – Documented losses: Up to $3,500 per person for documented fraud or identity theft losses related to the data incident. This option requires supporting documentation.
- Cash Payment B – Alternate cash: An estimated $75 flat payment for those who do not have documentation or do not wish to claim documented losses. This option does not require documentation.
- Medical data monitoring: In addition to either cash payment, class members may claim one year of free medical data monitoring.
The administrator will calculate the net settlement fund by deducting settlement administration costs, attorneys’ fees and costs, service awards and the cost of medical data monitoring from the $1.7 million fund. It will pay documented losses claims first, then alternate cash payments.
If the total amount class members claim is less than the net settlement fund, the settlement administrator may increase each claimant’s payment proportionally. If the total amount class members claim exceeds the net settlement fund, the administrator may reduce each payment proportionally.
How to claim an Outcomes One payment
Class members can file a claim online or download, print and complete the PDF claim form and mail it to the settlement administrator. The claim deadline is May 5, 2026.

If a class member's contact information changes after they submit their claim, they should notify the settlement administrator in writing at the address above.
What proof or documentation is necessary to submit a claim?
- Online claimants must provide the unique ID and PIN from the settlement notice they received in the mail. Those filling out and mailing the PDF claim form need only provide the unique ID. Class members who lost or did not receive their notice should contact the settlement administrator for assistance.
- Class members filing a documented losses (cash payment A) claim must submit reasonable documentation showing their loss is actual, documented, unreimbursed, most likely caused by the data incident and incurred after July 1, 2025. Examples include bills, invoices, receipts, account statements or correspondence related to the loss. Self-prepared documents alone are not sufficient.
- Class members filing an alternate cash payment (cash payment B) or medical data monitoring claim do not need to submit documentation but must select the appropriate option on the claim form.
