Oklahoma Surgical Hospital $1M Patient Privacy Settlement

Patients of Oklahoma Surgical Hospital who visited any of its websites, patient portals, social media sites or bill payment sites between July 1, 2021, and Nov. 5, 2023, may be eligible to claim $12 from a class action settlement.

Oklahoma Surgical Hospital LLC agreed to pay at least $1,000,000 to settle a class action lawsuit alleging it used tracking technologies on its websites that shared information about patient visits with third parties, such as Meta and Google, without patient knowledge or consent.

Who can file an Oklahoma Surgical Hospital claim?

Class members must have been a patient of Oklahoma Surgical Hospital between July 1, 2021, and Nov. 5, 2023, and visited any of the following during that period:

• Oklahoma Surgical Hospital’s website
• Oklahoma Surgical Hospital’s patient portal
• Oklahoma Surgical Hospital’s social media site
• Oklahoma Surgical Hospital’s bill payment site

Class members do not need to have received medical treatment during this time; simply visiting one of the hospital’s online platforms as a patient is sufficient to qualify. The settlement website estimates 55,664 potential class members.

How much can class members receive?

Each eligible class member who submits a valid claim will receive a payment of $12. Each class member may only submit one claim and receive one payment.

How to claim a data privacy class action rebate

Class members can file a claim online or download and print the PDF claim form%20Exhibit%20C%20-%20OSH%20-%20Claim%20Form%20v1.pdf), fill it out and mail it to the settlement administrator. Claims must be submitted online or postmarked by Dec. 26, 2025.

What information is required to submit a claim?

To file a claim, class members must provide the class member ID provided on their settlement notice.

Payout options

Payment will be issued by check and mailed to the address provided on the claim form. Any uncashed checks after 120 days will be donated to OK Kids Korral or another charity chosen by the court.

The $1,000,000 settlement fund covers:

• Service award to class representative: Up to $5,000
