Individuals who are a resident of the United States whose personal or health information was potentially accessed during the Integris Health data breach, may be eligible to submit claim for up to $25,000 or a cash award from a class action settlement.

Integris Health Inc. has agreed to pay $30 million to settle a class action lawsuit related to a cyberattack that occurred on or about Nov. 28, 2023. The class action claims that certain files containing personal information, including names, dates of birth, contact information, and Social Security numbers, were potentially accessed during the breach. The data incident affected an estimated 2.39 million individuals.

Who is eligible for a data breach payout?

Class members are individuals whose personally identifiable information (PII) and/or protected health information (PHI) was or may have been accessed or acquired in the 2023 Integris Health data security incident.

PII means personally identifiable information, and PHI means protected health information.

How much is the class action settlement payment?

Class members may submit to receive one or more of the following benefits:

  • Credit monitoring and insurance services: All class members elect to receive three years of three-bureau credit monitoring services, which includes up to $1 million in identity theft insurance.
  • Documented loss payment: Class members may claim reimbursement of up to $25,000 for actual, documented out-of-pocket losses traceable to the data breach. Covered expenses and losses include:
    • Credit monitoring or identity theft insurance purchased by the claimant after the breach occurred
    • Fees for credit reports or credit freezes
    • Costs for replacing payment cards or opening new bank accounts
    • Postage, phone charges, express mail and other incidentals
    • Unrefunded overdraft, late or missed payment fees
    • Unrefunded fraudulent charges occurring on or after Nov. 1, 2023
    • Damages from stolen benefits or tax returns
    • Services to remove your information from the dark web
    • Ransom payments related to the breach
  • Pro rata cash award: Class members that do not submit a documented losses claim, can file to receive a pro rata cash payment, estimated at $100. Final payment amount will be determined by the number of valid claims filed.

How to claim a class action rebate

To receive a settlement payment, a claim form must be submitted by Dec. 22, 2025. Class members have the following options:

  • File a claim online
  • Download and print a PDF claim form to mail to the settlement administrator
  • Email an electronic image of the completed claim form to the settlement administrator

Required proof and claim information

  • Unique ID and PIN from settlement notice required to submit an online claim form.
  • Documented loss payment claims require supporting documentation, such as bank or credit card statements showing fees or fraudulent charges, invoices for services, police report and other proof of identity theft.

  • PayPal
  • Venmo
  • Zelle
  • Virtual prepaid card
  • Physical check mailed to address provided

$30 million data breach settlement fund

The $30,000,000 settlement fund will include:

  • Settlement administration costs: To be determined
  • Attorneys’ fees: Up to $9,999,000
  • Attorneys’ expenses: Up to $75,000
  • Service awards to class representatives: $5,000 each, a total of $70,000
  • Credit monitoring services: Cost determined by the number of claims filed
  • Payments to approved claimants: Remaining settlement funds

When is the Integris Health data breach settlement payout date?

Payments and credit monitoring codes will be issued approximately 90 days after the court grants final approval of the settlement.

Why did this class action settlement happen?

This class action lawsuit alleges that Integris Health experienced a cyber incident in late 2023 that potentially exposed private information. Plaintiffs claim that Integris failed to adequately protect this data belonging to current and former patients.

Integris denies all allegations but agreed to settle to avoid the expense and uncertainty of a trial and appeals.
