
Current and former patients who received a notification from City of Hope National Medical Center in or after April 2024 regarding a data security incident involving their personal or health information may qualify to submit a claim for up to $5,000 from a class action settlement.
City of Hope National Medical Center agreed to pay $8.5 million to settle a class action lawsuit alleging it failed to implement and maintain reasonable security measures to protect private information, resulting in a data breach discovered in October 2023. The settlement class includes more than 800,000 individuals.
Who is eligible for a data breach payout?
Class members must meet the following criteria:
- They received a notice from City of Hope National Medical Center or the settlement administrator in or after April 2024 stating their personally identifiable information or protected health information may have been involved in the October 2023 data security incident.
- They received a settlement notice informing them of their eligibility to submit a claim.
How much is the City of Hope settlement payment?
Class members may claim one or more of the following benefits:
- Documented losses: Class members can submit a claim to receive reimbursement for actual, documented losses or expenses up to $5,000. Eligible losses include:
- Costs of credit reports purchased after Oct. 13, 2023
- Costs paid for credit monitoring or fraud resolution services after Oct. 13, 2023, if purchased primarily because of the breach
- Expenses directly associated with identity theft or fraud related to the breach
- Documented losses fairly traceable to the incident
- Compensation for up to four hours of lost time at $25 per hour for a total of $100
- Alternative cash payment: Class members who do not submit a documented losses claim may claim a pro rata cash payment estimated at $100. The settlement administrator will determine the final amount by the number of valid claims.
- California statutory cash payment: California residents, or those who lived in California during the class period, can claim an additional pro rata cash payment estimated at $250. The settlement administrator will determine the final amount by the number of valid claims.
- Medical identity, theft protection and credit monitoring services: All class members who submit a documented losses or alternative cash payment claim will automatically receive a code to enroll in CyEx medical information protection and credit monitoring services.
How to claim a class action rebate
To receive a data breach settlement payment, class members must submit a claim form by Jan. 13, 2026. They can submit an online claim form or print and mail the PDF claim form%20v2.pdf) to the settlement administrator.

Required proof and claim information
- All class members must provide the class Member ID from the official settlement notice they received.
- Documented loss claims require proof, such as receipts, bank statements, credit card statements, invoices for services, telephone records, police reports and other documentation showing fraudulent transactions traceable to the data breach.

The $8,500,000 settlement fund will include:
- Settlement administration costs: To be determined
- Attorneys’ fees: Up to $2,833,333.33
- Attorneys’ expenses: Up to $50,000
- Service awards to class representatives: Up to $4,000 per representative
- Credit monitoring services: Cost determined by the number of claims filed
- Payments to approved claimants: Remaining settlement funds

When is the City of Hope data breach settlement payout date?
The settlement administrator will issue payments to eligible class members approximately 70 days after the court grants final approval of the settlement.
