
Current and former patients who received a notice that the November 2023 Bradford Health Services and Bradford Health Partners data breach compromised their private information may qualify to submit a claim for up to $5,000 from a class action settlement.
Bradford Health Partners LLC and Bradford Health Services LLC agreed to pay $900,000 to settle a class action lawsuit claiming a cybersecurity incident resulted in unauthorized access to or acquisition of private and protected health information. This information could include names, dates of birth, Social Security numbers, medical details, insurance information and financial data belonging to 32,425 people.
Who can file a claim for a data breach payout?
Class members are individuals residing in the United States who received a received a notice from Bradford Health that the November 2023 data breach impacted their private information.
How much are settlement payments?
Class members can claim one of two cash payment options and may also receive medical data monitoring services.
- Cash payment A – Documented losses: Class members can claim up to $5,000 for documented out-of-pocket losses or expenses traceable to the data breach.
- Cash payment B – Alternate cash: Class members who do not submit a documented losses claim can submit a claim to receive a cash payment estimated at $150. The settlement administrator will determine the final payment amount by the number of claims filed.
- Medical data monitoring: All class members can elect to receive three years of free medical data monitoring services.
How to claim a class action rebate
To receive a settlement payment, class members can file a claim online or print the PDF claim form to complete and mail to the settlement administrator.

Required proof and claim information
- All class members must provide the unique ID from the settlement notice they received. Online claimants must also provide the PIN from the same notice.
- Documented losses claims require supporting documentation of expenses or losses. This may include receipts, invoices, bank or credit card statements showing unreimbursed fees or fraudulent charges, correspondence and other proof of identity theft or fraud.

Settlement fund breakdown
The $900,000 settlement fund will include:

- Attorneys’ fees: Up to $300,000
- Attorneys’ expenses: To be presented to the court for approval at a later date
- Service awards to class representatives: Up to $3,500 each
- Medical data monitoring: Cost determined by the number of claims filed
- Payments to approved claimants: Remaining settlement funds

When is the Bradford Health data breach payout date?
The settlement administrator will issue payments and medical monitoring benefits to approved claimants no later than 75 days after the court grants final approval of the settlement.
Why did this class action settlement happen?
This class action lawsuit claimed a cybersecurity incident in November 2023 resulted in unauthorized access to or acquisition of private information held by Bradford Health Partners LLC and Bradford Health Services LLC. The plaintiffs alleged the defendants failed to adequately protect sensitive data.
Bradford Health denies any wrongdoing but agreed to settle to avoid the risk and expense of continued litigation. The settlement agreement also requires the defendants to implement enhanced security measures.
