
Individuals who received a notification stating a January 2025 data breach at Alabama Ophthalmology Associates PC may have compromised their personal information could be eligible to claim up to $5,000 and/or credit monitoring from a class action settlement.
Alabama Ophthalmology Associates PC agreed to pay $850,000 to settle a class action lawsuit alleging it failed to adequately protect sensitive patient data during a targeted cyberattack.
Who can file a claim?
Class members must meet the following criteria:
- They reside in the United States.
- Alabama Ophthalmology notified them that the January 2025 data incident may have impacted their information.
The potentially compromised information includes Social Security numbers, health insurance details and treatment information.
How much can class members get?
Class members can claim one of the following benefits:
- Out-of-pocket expense reimbursement: Up to $5,000 for documented losses resulting from the data breach, including losses due to identity theft or fraud, costs to replace IDs, postage to contact banks or fees for credit reports, credit monitoring or freezing/unfreezing credit
- Alternative pro rata cash payment: In lieu of out-of-pocket expense reimbursement, a one-time payment estimated at $60 that does not require documentation. The actual amount may be higher or lower depending on the number of valid claims.
In addition, all class members can claim two years of CyEx Medical Shield Complete, which offers medical data monitoring and identity theft protection.
If the number of claims exceeds the available funds, the settlement administrator will adjust payments proportionally to ensure it can pay all valid claims.
How to claim a settlement payment
Class members can file a claim online or download and print a PDF claim form, complete it and mail it to the settlement administrator. The claim deadline is July 6, 2026.

What proof or documentation is required to submit a claim?
- To file an online claim form, class members must log in with the unique ID and PIN from the settlement notice they received. Those who are unable to locate their ID and PIN should contact the settlement administrator by emailing info@AOASettlement.com and providing their full name and mailing address.
- To file a claim for reimbursement of out-of-pocket expenses, class members must provide documentation to support their claim. Personal certifications or affidavits alone are not sufficient, but the class member may submit them as additional context.
- To file a claim for the alternate cash payment or credit monitoring, class members need only select the appropriate option on the claim form.
Payout options
- Class members who submit a claim by mail will receive a paper check at the address they provide.
- Class members who submit a claim online can choose to receive payment by PayPal, Venmo, Zelle or mailed check.

When is the Alabama Ophthalmology data breach settlement payout date?
