What happened?
On Feb. 25, 2025, Genex Services discovered suspicious activity on parts of its network and an investigation discovered a phishing attack allowed a cybercriminal to access files containing both personally identifiable information (PII) and protected health information (PHI).
The compromised files included data associated with individuals whose insurance claims Genex assisted with the processing of. Affected individuals have been notified by mail.

The breach was reported to the California Attorney General’s office on July 9, 2025, and to the Texas Attorney General’s office on July 10, 2025. The total number of impacted individuals has not been released but includes at least 373 in Texas.
