A recent data breach at Swift Institute has exposed sensitive personal and health information of patients and individuals associated with the Nevada-based medical practice. The incident was discovered Jan. 30, 2025, after the company detected unusual activity within its network environment. A subsequent investigation revealed that, on or about Jan. 28, 2025, certain files were copied by an unauthorized individual.

The breach was confirmed to have impacted a range of personal and protected health information including names, Social Security numbers, driver’s license or state identification card numbers, dates of birth, financial account information, credit or debit card information, medical information, health insurance information and digital signatures.

This means both personally identifiable information (PII) and protected health information (PHI) were affected. The company completed its review of the impacted data on Dec. 4, 2025, and began notifying affected individuals shortly after.

The breach was reported to the California Attorney General on Dec. 12, 2025. Additionally, Swift Institute has posted its own Notice of Data Event on its website.

Swift Institute's response

Swift Institute responded promptly upon discovering the breach, engaging legal counsel and third-party forensic specialists to investigate the scope and nature of the incident. As soon as the company confirmed that sensitive information had been accessed, it began notifying potentially affected individuals.

Swift Institute is offering at least 12 months of complimentary credit monitoring and identity theft restoration services to those impacted by the breach. These services are provided through Cyberscout, a TransUnion company, and include single-bureau credit monitoring, credit reports and credit scores. To activate these services, affected individuals must enroll within 90 days of receiving their notification letter.

Swift Institute is also providing guidance on how to place fraud alerts and security freezes on credit files, as well as information about obtaining free credit reports and remaining vigilant for incidents of identity theft or fraud. The company has set up a dedicated assistance line at 833-844-8151, available Monday through Friday from 8 a.m. to 8 p.m. Eastern time, to answer questions and provide support.

Given that both PII and PHI were involved in this breach, individuals are encouraged to closely monitor their financial accounts, review credit reports for suspicious activity and consider placing a fraud alert or credit freeze with the major credit bureaus. It is also recommended to notify financial institutions of the breach and to remain alert for any signs of identity theft or misuse of personal information.
