The practice disclosed the breach to the Texas Attorney General on Sept. 11, 2026. Affected patients were notified by U.S. Mail, and the practice posted a notice on its website.
On May 26, 2026, the practice was informed by its IT support company that irregularities had been identified related to remote access session hosts. The primary focus of the IT team shifted to locking down external access to the practice's remote desktop web services infrastructure and ensuring that all external connections to the system were terminated.
The types of information reported as potentially exposed included both personally identifiable information and protected health information, including names, addresses, dates of birth, Social Security numbers, driver's license numbers, government-issued identification numbers, financial information, medical information and health insurance information.
The practice is offering complimentary identity protection monitoring services at no cost to affected patients. Individuals should follow the instructions provided in their notification letter to enroll.
