Florida Physician Specialists, a multi-specialty medical practice based in Jacksonville, Florida, disclosed a data breach involving unauthorized access to its computer network between approximately Nov. 27 and Nov. 29, 2025.

A comprehensive investigation and document review followed to determine what information may have been affected. The investigation, which concluded on April 6, 2026, found that personal information may have been removed from the company's network during the three-day window of unauthorized access.

The types of personal information exposed included full names and one or more of the following: Social Security numbers, driver's license numbers, state identification numbers, other government identification numbers, financial account information, credit or debit card information, medical information and health insurance policy information.

The breach was first disclosed to the Maine Attorney General on April 25, 2026, with 47 Maine residents identified as affected. The company also posted a notice about the incident on its website.

Florida Physician Specialists' response to the breach

Florida Physician Specialists is offering affected individuals complimentary credit monitoring through Equifax Credit Watch Gold.

Affected individuals can enroll by visiting www.equifax.com/activate and entering the unique activation code included in their notification letter, which was mailed out on April 24, 2026.

The company has set up a dedicated toll-free response line at 844-5.58-4678 for individuals with questions about the incident. The line is staffed with professionals familiar with the breach and is available Monday through Friday from 9:00 a.m. to 9:00 p.m. Eastern Time.

Steps to take if your information was exposed

  • Place a fraud alert or credit freeze on credit files by contacting Equifax (1-800-525-6285), Experian (1-888-397-3742) and TransUnion (1-800-680-7289).
  • Request free credit reports at AnnualCreditReport.com and review them for unfamiliar accounts or inquiries that were not authorized.
  • Monitor financial account statements regularly for unauthorized transactions and review them carefully for any charges that seem unfamiliar.
  • Review Explanation of Benefits statements from health insurance companies for any services not received, since medical and health insurance information was involved in this breach.
  • Be cautious of phishing attempts that reference Florida Physician Specialists or this breach by name, as scammers often exploit breach notifications to steal additional information.
  • Report suspected identity theft to the Federal Trade Commission at ftc.gov/idtheft or by calling 1-877-438-4338, and file a police report if personal information has been misused.
