Doctor's Choice Data Breach Impacts 14,333 Texans

Doctor's Choice Home Care, a Houston-based home health care and hospice provider, disclosed a data breach connected to its electronic medical record vendor, WellSky.

WellSky is a third-party electronic medical record platform that health care providers like Doctor's Choice Home Care use to manage, store and coordinate patient records.

Between June 5, 2026, and July 24, 2026, an unauthorized party gained access to a single clinical user account within the WellSky electronic medical record system. The company discovered the incident on July 21, 2026.

A review of the company's own internal systems and networks found no related unauthorized activity, confirming that the breach was isolated to the WellSky platform.

The types of information potentially exposed included names, addresses, dates of birth, Social Security numbers, scheduling information, clinical and treatment information and health insurance information.

The breach affected 14,333 Texas residents. The company notified affected individuals by U.S. mail and through a notice posted on its website.

Doctor's Choice Home Care's response to the breach

The company directed affected individuals to request free credit reports from the three nationwide credit bureaus under the Fair Credit Reporting Act, and provided mailing addresses and phone numbers for Equifax, Experian and TransUnion.
