
Current and former patients who received a notice from Memorial Hospital and Manor that their private information may have been compromised in the November 2024 data breach, may be eligible to submit a claim for up to $5,000 from a class action settlement. The settlement has approximately 105,000 class members.
The Hospital Authority of the City of Bainbridge and Decatur County, doing business as Memorial Hospital and Manor, has agreed to pay to settle a class action lawsuit. The lawsuit claims that a targeted cyberattack in November 2024 led to unauthorized access to files containing sensitive personal and health information.
Who can file a claim for a data breach payout?
Class members are individuals in the United States who received notice from Memorial Hospital and Manor that their private information may have been compromised in the November 2024 data incident.
How much is the class action settlement payment?
Class members can claim one of the following cash payment options and medical data monitoring:
- Option A – Documented losses cash payment: Class members can claim up to $5,000 in actual, documented out-of-pocket losses that occurred between Nov. 2, 2024, and Jan. 5, 2026. Losses include fees for credit monitoring, costs to replace IDs, postage to contact banks and other monetary losses due to identity theft or fraud.
- Option B – Cash payment for lost time: Class members can claim up to four hours of time spent time addressing issues related to the data incident, at $25 per hour, a maximum of $100. Lost time may include changing passwords, investigating suspicious activity or researching the incident.
- Option C – Alternate cash payment: Class members who do not submit a claim for documented losses or lost time, can submit to receive a flat $40 cash payment.
- Medical data monitoring: All class members can elect to receive one year of CyEx Medical Shield Pro. Services provided includes $1 million in medical identity theft insurance and monitoring for healthcare insurance ID exposure, medical record number exposure, and unauthorized health savings account spending.
How to claim a data breach class action rebate
To receive a settlement payment or free medical data monitoring, a claim form must be submitted. Class members can file a claim online or print the PDF claim form to complete and mail to the settlement administrator.

The claim form must be submitted online or postmarked by Jan. 5, 2026.
Required proof and claim information
- Unique ID and PIN from official settlement notice required to submit an online claim form.
- Documented losses claims must include supporting documentation such as receipts, invoices, bank statements, or other records showing identity theft and unreimbursed fraudulent charges.
- Lost time claims must include a brief written description of the time spent and actions taken.

Data breach settlement fund
The settlement fund will include:
- Settlement administration costs: To be determined
- Attorneys' fees and costs: Up to $500,000
- Service awards to class representatives: $1,500 each
- Medical data monitoring: Cost determined by number of claims filed.
- Payments to approved claimants: Total determined by the number of valid claims submitted.

When is the Memorial Hospital and Manor data breach settlement payout date?
Payments and medical monitoring codes will be issued to claimants no later than 85 days after the settlement is granted final approval.
Why was there a class action settlement?
This class action lawsuit was filed after a targeted cyberattack on Memorial Hospital and Manor’s internal network in November 2024. The incident allegedly resulted in unauthorized access to files containing private information, including names, dates of birth, Social Security numbers, and protected health information.
Memorial Hospital and Manor denies the allegations, but agreed to settle to avoid the expense and uncertainties of continued litigation.
