Consumers who received a notification from Acadia Health LLC d/b/a Just Kids Dental about a data breach discovered in or around August 2023 may qualify to claim up to $10,000 from a class action settlement.

Who is eligible to file a Just Kids Dental claim?

Class members must meet the following criteria:

  • They received a notification letter from Acadia Health d/b/a Just Kids Dental stating their private information was or may have been compromised in the August 2023 data breach.
  • Their name appears on the defendant’s settlement class list, which includes all individuals whose information may have been exposed in the incident.

The class is divided into three subclasses:

  • Minor Subclass: Individuals who were minors at the time of the data breach
  • Adult Subclass: Individuals who were adults at the time of the data breach
  • SSN Subclass: Individuals whose Social Security Numbers were involved in the breach

How much is the data breach settlement payout?

  • Minor Subclass: This class automatically receives up to 10 years of Cy Ex’s Minor Defense Pro product (or a similar product) for credit and identity monitoring.
  • SSN Subclass: This class may claim:
    • Two years of three-bureau credit monitoring and identity protection services, including $1 million in identity fraud insurance
    • Up to $10,000 in documented losses (ordinary and extraordinary) traceable to the data breach
    • An alternative pro rata cash payment
  • Adult Subclass (not in SSN Subclass): This class may claim:
    • Up to $2,500 in documented ordinary losses traceable to the data breach
    • An alternative pro rata cash payment

How to claim a Just Kids Dental class action rebate

To receive a settlement payment, eligible class members must file a claim online or download and print a PDF claim form to complete and mail to the settlement administrator. The claim form deadline is Aug. 6, 2025.

Proof and documentation

  • Claimants must provide the Class Member ID found on their postcard notice. To file an online claim, they also need their PIN found on the same notice.
  • For reimbursement of documented losses, claimants must provide receipts or other non-self-prepared documentation showing actual out-of-pocket expenses and a description of how the loss is related to the data breach. These ordinary losses may include unreimbursed bank fees, credit monitoring costs, postage or gasoline for local travel.
  • For extraordinary losses (SSN subclass only), claimants must provide documentation showing the loss was actual, documented, unreimbursed related to the data breach and that they made reasonable efforts to recover the loss. Extraordinary losses may include costs, expenses, losses or charges resulting from identity theft or fraud, falsified tax returns or other misuse of private information.
