Community Dental Care Data Breach Class Action Settlement

Current and former dental patients who received notice that their personal information was compromised in the December 2024 Community Dental Care data breach, may qualify to submit a claim for up to $5,000 from a class action settlement.

Community Dental Care, a Minnesota-based nonprofit dental services provider, has agreed to settle a class action lawsuit alleging the company failed to adequately protect patient information during a targeted cyberattack in December 2024. The cybersecurity incident may have compromised names, health insurance details, dates of birth, medical information and Social Security numbers belonging to approximately 130,583 individuals.

Who can file a claim for a data breach payout?

Class members are individuals whose personal information was accessed without authorization in the December 2024 Community Dental Care data breach, including those who received notice of the incident.

How much are settlement payments?

Class members have the following benefit options:

• Cash payment A - Documented losses: Class members can claim up to $5,000 for unreimbursed, documented out-of-pocket losses that occurred between Dec. 20, 2024 and Oct. 19, 2026 and were caused by the data breach. This includes monetary losses due to identity theft or fraud, fees for credit reports, credit monitoring, or freezing/unfreezing credit, cost to replace IDs and postage to contact financial institutions.
• Cash payment B - Alternative cash: Class members that do not submit a documented losses claim can submit to receive a one-time $50 cash payment.
• Medical and credit monitoring: All class members can elect to receive two years of CyEx Medical Shield Complete. Services include $1 million of medical identity theft insurance, access to a fraud resolution agent if needed and monitoring for healthcare insurance ID and medical record number exposure and unauthorized HSA spending. A claim form is not required to receive this claim. Enrollment codes have been sent to all class members by postcard.

To receive a settlement payment payment, class members can file a claim online or print and complete the PDF claim form and mail or email it to the settlement administrator.

• The LoginID and PIN from the settlement notice are required to submit a claim online or by mail.
• Documented losses claims require supporting documentation such as receipts, bank or credit statements or other proof of identity theft or fraud caused by the breach and not reimbursed by a third party.

Settlement fund breakdown

When is the Community Dental Care data breach settlement payout date?

Payments will be issued to approved claimants approximately 60 days after the court grants final approval of the settlement.

Why did this class action settlement happen?

The class action lawsuit was filed after a targeted cyberattack in December 2024 compromised the private information of Community Dental Care patients. Plaintiffs alleged that Community Dental Care failed to protect sensitive data, resulting in unauthorized access to personal and medical information.

Community Dental Care denies the allegations, but agreed to settle to avoid the expense and risk of further litigation.
