CBiz Benefits & Insurance Services Inc. agreed to pay as much as $485,000 to settle a class action lawsuit alleging it failed to implement and maintain reasonable security measures to protect personal information, resulting in a 2024 cybersecurity incident.

Who can file a claim for a data breach payout?

Class members are individuals who received a notice from CBiz indicating the 2024 data breach may have compromised their private information.

How much are settlement payments?

Class members can claim one or both of the following:

  • Out-of-pocket losses: Class members can claim up to $5,000 for documented out-of-pocket losses or expenses that occurred in or after 2024 and are traceable to the data breach. These may include monetary losses from fraud or identity theft; professional fees for attorneys, accountants or credit repair services; costs to freeze/unfreeze credit; credit monitoring costs after the incident; and postage for contacting financial institutions.
  • Pro rata cash payment: All class members can also submit a claim to receive a pro rata cash payment estimated at $100. The settlement administrator will determine the final payment amount by the total number of claims filed.

How to claim a class action rebate

To claim a settlement payment, class members can file a claim online or print and complete the PDF claim form and mail it to the settlement administrator.

Required proof and claim information
