Individuals who received a notice that the ApolloMD data breach may have compromised their private information could qualify to submit a claim for up to $5,000 and/or credit monitoring from a class action settlement. The cybersecurity incident impacted more than 626,000 current and former patients from multiple medical provider practices.

ApolloMD Business Services LLC agreed to pay $4.02 million to settle a class action lawsuit claiming a data security incident that occurred in May 2025 compromised personally identifiable and protected health information, resulting in unauthorized access to sensitive patient data.

Who can file a claim for a data breach settlement payout?

Class members are individuals residing in the United States who received a notice regarding the ApolloMD data breach, indicating the incident may have impacted their private information.

How much are settlement payments?

Class members have the following benefit options:

  • Cash payment A – documented losses: Class members can claim up to $5,000 for reimbursement of reasonable documented losses that occurred due to fraud or identity theft related to the data breach.
  • Cash payment B – alternate cash: Class members who do not submit a documented losses claim can submit a claim to receive an estimated $75 pro rata cash payment. The settlement administrator will determine the final payment amount by the number of claims filed.
  • Medical data monitoring: All class members can elect to receive one year of CyEx’s medical data monitoring, which includes real-time one-bureau credit file monitoring, dark web scanning, security freezing assistance, victim assistance, $1 million in identity theft insurance with no deductible and access to fraud resolution agents.

How to claim a class action rebate

To receive a settlement payment, class members can file a claim online or print the PDF claim form to complete and mail to the settlement administrator.

Required proof and claim information

  • All class members must provide the class member ID from the settlement notice to submit a claim online or by mail.
  • Documented losses claims require supporting documentation, which may include receipts, invoices, bank or credit card statements showing unreimbursed fees or fraudulent charges and other proof of identity theft or fraud.

When is the ApolloMD data breach settlement payout date?

The settlement administrator will issue payments to approved claimants approximately 75 days after the court grants final approval of the settlement.

Why is there a class action settlement?

The class action lawsuit claimed ApolloMD discovered suspicious activity in its IT network in May 2025. An investigation revealed an unauthorized third party may have accessed or acquired files containing sensitive patient information. The plaintiffs alleged ApolloMD failed to adequately safeguard personal patient data, leading to claims of negligence and breach of implied contract.

ApolloMD denies the allegations but agreed to settle to avoid the uncertainty and cost of further litigation and a possible trial.
