On July 30, 2025, Triad Radiology Associates, a leading independent radiology practice based in Winston-Salem, N.C., identified suspicious activity involving an employee’s email account.

An investigation revealed that between July 11 and Sept. 8, 2025, unauthorized access to the email account may have occurred, potentially exposing sensitive information.

The breach appears to have originated from unauthorized access to a single employee’s email account, though there is no indication at this time of who was responsible for the intrusion.

A thorough review determined that the types of information exposed varied by individual. The data potentially accessed included names, addresses, Social Security numbers, driver’s license numbers, bank account information, dates of birth, medical information and health insurance details.

The company has not disclosed the exact number of individuals affected. Triad Radiology Associates posted a notice of the incident to its website per a clickable banner.

Individuals who believe they may be impacted are encouraged to enroll in these services by calling 1-800-405-6108, Monday through Friday, from 8 a.m. to 8 p.m., excluding holidays. Written inquiries can be sent to 3010 Trenwest Drive, Winston-Salem, NC.

Affected individuals should remain vigilant by regularly reviewing credit reports, account statements and explanation of benefits forms for suspicious activity. Placing a fraud alert or credit freeze with the major credit reporting agencies—TransUnion, Experian and Equifax—can provide additional protection.
