What Happened?
On or around July 30, 2025, Triad Radiology Associates discovered suspicious activity involving an employee’s email account. Action was taken to secure the account, and third-party specialists were brought in to investigate.
The investigation determined that certain information may have been accessed between July 11, 2025, and Sept. 8, 2025. A comprehensive review found that the type of information exposed varies by individual.

- Addresses
- Social Security numbers
- Driver’s license numbers
- Bank account information
- Dates of birth
- Medical information
- Health insurance information
