The breach was reported to the Texas Attorney General on Sept. 3, 2026, with 2,065 Texas residents identified as affected.
The personally identifiable information exposed in the breach included names, addresses, dates of birth, Social Security numbers, driver's license numbers and government-issued identification numbers such as passports or state ID cards.
The breach also exposed protected health information, including medical information and health insurance information.
