Killeen-based, Integrated Pain Associates, a practice serving Central and West Texas, disclosed on its website a data breach that occurred in February 2026.
The company posted a notice on its website on April 30, 2026, alerting patients and the public to the incident.
Integrated Pain Associates recently identified suspicious activity on its computer systems, according to the company's notification. An investigation into the incident determined that certain information stored on the company's systems may have been subject to unauthorized access or acquisition on or around Feb. 24, 2026.
The public notice was issued roughly two months after the breach occurred.
The company stated it is providing notification "in an abundance of caution" while the investigation into the full scope of the incident continues. The ongoing review includes determining what specific information may have been affected and which individuals may need to be notified.
The types of information believed to be potentially at risk include individuals' first and last names together with one or more of the following: addresses, dates of birth, driver's license numbers, Social Security numbers, diagnosis or condition information, medication information, health insurance information, provider names, other treatment information and financial account information. Not all of these data types were necessarily exposed for every individual, according to the notification.
Integrated Pain Associates notified law enforcement about the breach and stated it is reviewing its existing policies and procedures related to data protection. Additionally, the company said it has implemented additional security measures to further minimize the risk of a similar incident occurring in the future.
Because the breach may have exposed highly sensitive information, including Social Security numbers, financial account details and protected health information, the company is offering affected individuals access to credit monitoring and identity protection services at no cost. These services are being offered as an added precaution to help individuals monitor for potential misuse of their information. Individuals are encouraged to take advantage of these offerings given the serious nature of the data potentially exposed.
Individuals who have questions about the incident or who would like to enroll in the free credit monitoring and identity protection services can call 1-800-405-6108. The call center is available between 8 a.m. and 8 p.m. Eastern time, Monday through Friday, excluding holidays. Individuals may also reach the company by writing to 3800 S W S Young Drive, Suite 301, Killeen, Texas 76542.
