Green Imaging LLC, a virtual medical imaging network based in Houston, disclosed a data breach that may have exposed sensitive personal and medical information belonging to certain current and former patients.

The breach involved unauthorized access to a single employee email account within Green Imaging's email system over a 10-day period between Oct. 7, 2025, and Oct. 17, 2025. During this window, information stored in the compromised email account may have been viewed or copied by an unauthorized party.

After detecting the suspicious activity, the company launched an investigation with the assistance of third-party cybersecurity specialists to determine the nature and scope of the event. The investigation confirmed that only one email account was affected and that the company's electronic medical records database was not impacted by the breach.

Green Imaging then initiated a comprehensive third-party review of the contents of the affected email account to identify the types of information at issue and the individuals to whom it related.

The types of information present in the compromised account varied by individual and may include both personally identifiable information and protected health information: names, addresses, dates of birth, Social Security numbers, driver's license numbers, other government-issued identification numbers, clinical and treatment information, diagnosis or condition details, procedure types, physician information, medication details and other health or health insurance information.

At this time, 5,016 Texas residents were identified as affected by the breach. The company also posted a notice on its website providing information about the event and resources for those who may be impacted.

Green Imaging began notifying affected individuals by U.S. mail.

Green Imaging's response to the breach

Green Imaging encourages affected individuals to remain vigilant against incidents of identity theft and fraud. The company recommended reviewing account statements, credit reports and explanations of benefits for unusual activity.

Green Imaging has set up contact channels for individuals who believe they may be affected or who have additional questions about the incident. Those individuals can reach the company toll-free at 1-844-403-4597, by email at leaglteam@greenimaging.net, or by mail at 2020 Albans Road, Houston, TX 77005.

Steps to take if your information was exposed

  • Place a fraud alert or credit freeze with the three major credit bureaus: Equifax (1-888-298-0045), Experian (1-888-397-3742) and TransUnion (1-833-799-5355) to help prevent new accounts from being opened without authorization.
  • Request free credit reports at AnnualCreditReport.com and review them carefully for unfamiliar accounts, hard inquiries or other signs of misuse.
  • Check Explanation of Benefits statements from health insurers for any medical services, prescriptions or procedures not received, since protected health information was among the data types potentially exposed in this breach.
  • Monitor bank accounts and financial statements closely for unauthorized transactions, given that Social Security numbers and government-issued identification numbers were among the data that may have been compromised.
  • Watch for phishing attempts that reference Green Imaging or this data breach by name, as scammers sometimes use breach notifications to trick individuals into sharing additional personal details.
  • Report suspected identity theft to the Federal Trade Commission at IdentityTheft.gov or by calling 1-877-438-4338 if any suspicious activity is detected.
