Elevation Hospice of Utah LLC, a for-profit hospice care provider based in Draper, Utah, disclosed a data breach involving unauthorized activity detected in its email system in 2025.

Elevation Hospice of Utah is affiliated with Dallas-based Three Oaks Hospice Inc., which announced the breach on behalf of several affiliated hospice providers.

The breach was disclosed to the Texas Attorney General on Sept. 17, 2026, with 437 Texas residents identified as affected. The company additionally posted a notice about the incident on its website.

The breach involved unauthorized activity in Three Oaks Hospice's email environment. After the unauthorized email activity was first detected, Three Oaks Hospice stated that it took steps to investigate the scope and nature of the incident and determine what information may have been affected.

On July 20, 2026, Three Oaks Hospice determined that the unauthorized activity in its email system had affected personal information belonging to individuals connected to its affiliated hospice organizations.

On Aug. 17, 2026, Three Oaks Hospice identified the specific affiliated hospice organizations whose data was involved. In addition to Elevation Hospice of Utah, the affected affiliates named in the notification include Agape Hospice Care, Elevation Hospice of Colorado and Sage Hospice.

The types of information potentially exposed included names, dates of birth, driver license numbers, Social Security numbers, medical information and health insurance information.

Three Oaks Hospice's response to the breach

Affected individuals should remain alert to signs of identity theft or fraud. Individuals who receive a notification letter from Three Oaks Hospice should take the situation seriously and act promptly to protect their personal information.

Three Oaks Hospice established a dedicated toll-free call center for individuals who believe they may have been affected by the breach or who have questions about the incident. The call center can be reached at 1-800-610-8965 and is available from 7 a.m. to 7 p.m. Central Time, Monday through Friday, excluding major U.S. holidays.
