On June 11, 2025, Eleos Wellness discovered a data security incident that affected certain systems within its network environment. The organization, which provides behavioral health services in Pinellas Park, Florida, launched an investigation with the help of third-party cybersecurity professionals to determine the scope of the breach and whether sensitive information had been accessed or acquired by unauthorized parties.

At this time, the investigation is ongoing. However, Eleos Wellness has confirmed that information on the impacted systems may include both personally identifiable information (PII) and protected health information (PHI) including names, addresses, dates of birth, Social Security numbers and health insurance information. The company has stated there is no evidence that its cloud-based electronic health record system was compromised as a result of this incident.

Eleos Wellness has set up a dedicated, toll-free response line at 877-495-0947, available Monday through Friday from 9 a.m. to 9 p.m. Eastern time, to answer questions and provide additional information to those who may be affected.

The organization recommends that individuals potentially impacted by the breach remain vigilant by regularly reviewing their financial account statements for any signs of fraudulent activity. They also advise clients to carefully review explanation of benefits statements from their health insurance providers and to follow up on any unfamiliar items.

To help protect against identity theft, Eleos Wellness encourages affected individuals to:

  • Place a fraud alert on their credit files by contacting any of the three major credit bureaus (Equifax, Experian, or TransUnion)
  • Consider placing a security freeze on their credit reports, which restricts access to credit files and can prevent unauthorized accounts from being opened
  • Obtain free credit reports from AnnualCreditReport.com and check for discrepancies or unfamiliar accounts
  • Monitor health insurance statements for unrecognized services or providers

The company has also provided information about additional resources and steps for protecting health and financial information, as well as contact details for state attorney general offices for those seeking further guidance.
