Diversified Services Enterprises Inc., a provider of support management services to health care organizations based in Sarasota, Florida, disclosed a data breach that affected hundreds of individuals in the United States.

The breach was reported to the U.S. Department of Health and Human Services on June 13, 2025. The company also posted a notice about the incident on its website, which was originally dated June 13, 2025, and later updated on Sept. 10, 2026.

On April 14, 2025, DSE became aware of suspicious activity within its computer network. The company took steps to secure its environment and launched an investigation with the assistance of third-party forensic specialists to determine the nature and scope of the activity.

The investigation determined that between March 14, 2025, and April 16, 2025, an unauthorized party accessed and copied certain files stored on DSE's network.

After identifying the scope of the intrusion, DSE engaged a separate data review vendor to examine the contents of the affected files.

The personally identifiable information exposed included dates of birth, driver's license numbers or state identification card numbers, Social Security numbers, passport numbers, taxpayer identification numbers, financial account information and payment card information.

The protected health information exposed included medical record numbers, Medicare/Medicaid IDs, health insurance information and medical history information.

Diversified Services Enterprises' response to the breach

The company encouraged individuals to remain vigilant against incidents of identity theft and fraud by monitoring their accounts and credit reports for suspicious activity.

DSE set up a dedicated assistance line for anyone with questions about the incident. Individuals can call 844-301-0075 between 9 a.m. and 6:30 p.m. Eastern Time, Monday through Friday, excluding major U.S. holidays. Individuals who did not receive a notification letter but want to know if they were affected may also call this number to inquire.
