
Doxa Insurance Holdings LLC, a specialty insurance platform and holding company based in Fort Wayne, Indiana, disclosed a data breach involving sensitive personal information.
Doxa Insurance Holdings experienced a cybersecurity incident that impacted data the company maintains in its electronic systems. The letter did not provide specific dates for when the incident began, how long it lasted or when it was discovered.
The types of personal information exposed in the breach included Social Security numbers, driver's license numbers and government-issued identification numbers.
The breach was disclosed to the Massachusetts Office of Consumer Affairs and Business Regulation, as well as to the Vermont Attorney General. The company began mailing notification letters to affected consumers on July 27, 2026.
The total number of individuals affected across the United States remains undisclosed.
Doxa is offering affected individuals 24 months of complimentary identity theft protection and credit monitoring services.
To enroll, individuals must visit the Cyberscout activation page and enter the unique code included in their notification letter. Enrollment must be completed within 90 days of the letter's date.
Doxa has also set up a dedicated help line for individuals who have questions about the incident. Affected consumers can call 1-833-851-9219 and provide their unique code to speak with a fraud specialist. Representatives are available Monday through Friday from 8 a.m. to 8 p.m. Eastern time, excluding holidays. The help line will remain active for 90 days from the date of the notification letter.
