Consumers who received a notice indicating their private information may have been impacted by a data breach involving MD Now Medical Centers Inc. may qualify to claim up to $5,000 from a class action settlement.

  • They received a notice from MD Now Medical Centers Inc. stating their private information may have been impacted by the data breach discovered on or about May 20, 2024.

How much is the data breach payout?

Out-of-pocket expense reimbursement: Up to $5,000 for documented losses related to the data incident and not otherwise reimbursed by another source, such as insurance or credit monitoring services. Qualifying losses include:

  • Unreimbursed losses relating to fraud or identity theft
  • Professional fees (such as for lawyers, accountants or credit repair services)
  • Costs associated with freezing or unfreezing credit
  • Miscellaneous expenses like notary, postage and mileage

How to claim a class action rebate

Class members can file a claim online or download and print the PDF claim form to complete and mail to the settlement administrator. The claim deadline is Oct. 3, 2025.

When is the Herman v. MD Now Medical Centers settlement payout date?
