About WindRose Health Network

WindRose Health Network is a nonprofit community health center based in Indiana. Founded in 1996, it began as a rural health center in Trafalgar and has since expanded to multiple locations across southern Indianapolis, Johnson and Bartholomew counties.

As a federally qualified health center, WindRose Health Network provides a range of primary care services, including family medicine, pediatrics, convenient care, behavioral health and chiropractic care.

What Happened?

On Aug. 4, 2026, WindRose Health Network discovered a security incident involving its computer network.

The breach was caused by a previously undisclosed vulnerability in a remote-management tool used by one of its vendors. This tool allowed access to some of WindRose’s systems.

An unauthorized third party exploited this vulnerability and accessed one of WindRose’s servers between Aug. 3 and Aug. 4, 2026. The systems hosting electronic medical records were not affected.

The investigation revealed that files containing sensitive patient information were accessed.

Information Exposed:

  • Patient names
  • Patient identification numbers
  • Health insurance information
  • Dates of service
  • Names of medical providers

The breach affected 33,158 individuals in the United States.
