What Happened?
In October 2025, Unlimited Technology Systems discovered unauthorized activity within its commercial datacenter.
The company detected the breach on Oct. 19, 2025, and determined that an unauthorized actor may have accessed and obtained copies of personal information between Oct. 5 and Oct. 10, 2025.
An investigation revealed that the following types of consumer information were exposed:
Information Exposed:
- Names
- Health insurance and patient balance information (such as insurance policy numbers or claims/benefits information)
- Medical information (such as medical record numbers, dates of service and diagnosis information)
- Scanned documents (such as driver’s licenses or other government identification, insurance cards and intake forms)
- Social Security numbers
- Other personal information (such as dates of birth, emails, addresses, phone numbers or demographic information)
The company stated that full patient medical records, medical imaging and financial account information like credit card or bank account numbers were not included in the breach.
