The organization began an investigation, conducted forensic analysis, and removed the unauthorized party’s access. By Nov. 18, 2025, it was discovered that protected health information may have been involved.
The hacking group CL0P Ransomware claimed responsibility for the attack, posting about it on the dark web on Nov. 18, 2025.

- Diagnosis or treatment information
- Prescription information
- Medical record numbers
- Health insurance numbers
- Dates of service or evaluation
- Driver’s license numbers
- Full or partial Social Security numbers
A total of 6,556 people in the United States were affected. The breach was officially disclosed to the U.S. Department of Health and Human Services on Jan. 15, 2026.
