What Happened?
On or about Aug. 21, 2025, CHCP discovered suspicious activity on certain computer systems within its network.
An investigation revealed that an unauthorized third party accessed a limited portion of the network between Aug. 16 and 20, 2025. Certain files may have been accessed or downloaded without authorization.
By Jan. 30, 2026, it was determined that personal information was potentially included in the files accessed during the breach.
Information Exposed:
- Names
- Addresses
- Social Security numbers
- Driver’s license numbers or government-issued identification numbers
- Financial account information
- Medical information
- Health insurance information
At this time, it is undisclosed how many individuals were affected as a result of the breach.
