What happened?
On Jan. 30, 2025, Swift Institute discovered unusual activity in its network environment, which prompted an immediate investigation. The company learned that certain files may have been copied by an unauthorized individual on or about Jan. 28, 2025. After a thorough review, Swift Institute confirmed on Oct. 14, 2025, that sensitive personal and protected health information was involved.
Which types of information were exposed?
The following information may have been exposed in the breach:
- Names
- Social Security numbers
- Driver’s license or state identification card numbers
- Dates of birth
- Financial account information
- Credit or debit card information
- Medical information
- Health insurance information
- Digital signatures
Swift Institute began notifying affected individuals by mail and offered at least 12 months of complimentary credit monitoring and identity theft restoration services. The breach was disclosed to the California Attorney General’s office on Dec. 12, 2025.
