What Happened?

On Nov. 17, 2025, Patriot became aware of suspicious activity on their computer systems, which was later determined to be the result of a phishing email attack.

An employee email account was compromised, and unauthorized access to certain patient information occurred.

The company changed the password for the impacted account and engaged a third-party cybersecurity firm to investigate. The forensic investigation concluded on Jan. 9, 2026, confirming that specific emails and files were accessed without authorization.

After identifying the affected individuals and data, the company began mailing individual notice letters.

The breach was disclosed to the U.S. Department of Health and Human Services on Jan. 16, 2026, and impacted 1,415 people in the United States. Additionally, a notice of the incident was posted to the company's website.
