What Happened?

On or about June 3, 2025, Southwest Care Center experienced a cybersecurity incident involving a ransomware attack.

The MEDUSA ransomware group claimed responsibility, stating they accessed and intended to publish 143.9 GB of data from the organization on the dark web. The breach was publicly disclosed on June 27, 2025, when the group posted about the hack on the Tor network.

After discovering the incident, Southwest Care Center engaged third-party forensic specialists to secure their systems and investigate the extent of unauthorized activity. On Dec. 18, 2025, it was determined that an unauthorized third party may have acquired certain personal information.

Written notice was sent to all impacted individuals. Additionally, the center posted a notice of the incent on its website.
