The data breach affected thousands of residents, patients and staff at multiple Life Care Services facilities.

Regency Oaks reported the breach to the Massachusetts Attorney General’s office on July 12, 2025, and to the Vermont Attorney General’s office on July 14, 2025. Affected individuals were notified by mail beginning on July 11, 2025.
