What happened?
Rectangle Health discovered a data breach on Sept. 5, 2025, after being notified by Salesforce, the provider of a software platform used to store customer information. The unauthorized access occurred between Aug. 12 and Aug. 17, 2025. After investigating, Rectangle Health determined that sensitive personal information belonging to at least 2,095 individuals may have been impacted.

Rectangle Health began notifying affected individuals by written notice on Oct. 8, 2025. The cyberattack was also disclosed to the Maine Attorney General's office, the Massachusetts Attorney General's office and the New Hampshire Attorney General on Oct. 8, 2025.
