What Happened?

PIH Health, Inc. experienced a significant data breach between Nov. 14, 2024, and Dec. 2, 2024. On Dec. 1, 2024, the company discovered unusual activity within its digital environment. After securing the network, PIH Health launched an investigation and confirmed that an unauthorized actor had accessed certain files containing personal information.

A threat actor known as Dreamer2000 claimed responsibility for leaking PIH Health data and posted about the incident on the open web on Dec. 13, 2024. The breach was reported to the California Attorney General's office on Feb. 27, 2026.
