What Happened?
On Oct. 10, 2025, Ochsner LSU Health – Regional Urology identified unusual activity on retired Regional Urology systems that had not been in use since 2022. An investigation, supported by external cybersecurity experts, determined that an unauthorized third party gained access to these legacy systems and, around Oct. 5, 2025, obtained copies of certain files.
The breach affected the protected health information of at least 4,519 individuals in the United States.

- Medical record numbers
- Provider names
- Date(s) of treatment
- Medical history
- Imaging
- Procedure information
