The investigation determined that between Nov. 6, 2025, and Nov. 29, 2025, files containing personal information may have been accessed and acquired by an unauthorized party.
The CHAOS ransomware group claimed responsibility for the attack and posted about the breach on the dark web on Dec. 11, 2025, stating they had obtained 500 GB of data from the company.

- Driver’s license numbers or state identification numbers
- Other government identification numbers
- Financial account information
- Credit or debit card information
- Medical information
- Health insurance policy information
NSE Insurance Agencies began notifying affected individuals by U.S. Mail and through a notice on their website on Sept. 21, 2026.
