The investigation revealed that an unauthorized actor accessed the network between Jan. 15 and Jan. 18, 2026, and obtained certain files during that period.

A comprehensive review is ongoing to identify all individuals whose information was involved. New Horizons Behavioral Health has stated that written notifications will be mailed to affected individuals once the review is complete.
