The incident was reported to the U.S. Department of Health and Human Services on Feb. 26, 2026, and to the Massachusetts Attorney General’s office on Feb. 27, 2026. Additionally, Meadowlark Hills posted a public notice of the incident on its website and notified affected individuals.

- Driver’s license numbers or state identification numbers
- Other government identification numbers
- Financial account information
- Credit or debit card information
- Medical information
- Health insurance policy information
A total of 14,442 people in the United States were affected by this breach, including at least one Massachusetts resident.
