What Happened?
On May 21, 2025, Mainline Health Systems Inc discovered a data breach that occurred on or about April 10, 2024. Exposed patient information included personally identifiable information (PII) and protected health information (PHI).
The cybersecurity incident affected 101,104 individuals in the United States. Mainline began notifying impacted patients on June 20, 2025 and published a Notice of Data Security Incident on its own website.
