Legacy Health reported a data breach that was disclosed to the U.S. Department of Health and Human Services on Oct. 23, 2025, and the Texas Attorney General on Oct. 24, 2025.
According to the report, the breach affected at least 6,547 U.S. residents, including 4,031 people in Texas. The company notified affected individuals by U.S. Mail.
