What Happened?
On Jan. 15, 2026, Heart Care Centers of Illinois discovered suspicious activity related to an employee’s email account while investigating an unsuccessful phishing attempt.
A third-party forensic investigation revealed that a successful phishing attack had occurred, resulting in unauthorized access to the employee’s account between Aug. 22, 2024, and Nov. 6, 2024. The review determined that sensitive information may have been accessible to the unauthorized actor during this period.
The exposed information varies by individual but may include the following sensitive personal and health data:

Heart Care Centers of Illinois mailed notification letters to affected individuals beginning July 10, 2026.
