What Happened?

On Jan. 30, 2026, HIA discovered suspicious activity involving a user’s email account.

An internal investigation, supported by a third-party forensic firm, revealed that an unauthorized party had gained access to certain systems between Jan. 28, 2026, and Jan. 30, 2026. This included the compromised email account and a limited number of files.

The organization completed a detailed review on March 20, 2026, to determine the information involved and the individuals impacted.

  • Dates of service
  • Locations of service
  • Treatment information
  • Disability verification information
  • Medications
  • Social Security numbers

The breach was reported to the U.S. Department of Health and Human Services on March 30, 2026. In total, 1,143 people in the United States were affected.
