What happened?
On Jan. 28, 2025, Health Services LLC became aware of suspicious activity on its network. An investigation took place, and it was determined that between Jan. 2, 2025, and Jan. 28, 2025, an unauthorized actor accessed certain company systems and files containing sensitive information belonging to patients and potential patients.
Health Services LLC published a Notice of Data Security Incident on its website on March 31, 2025.
Information Exposed:
- Name
- Contact information, including phone number, email address and postal address
- Date of birth
- Driver’s license number
- Social Security number
- Patient ID
- Medical treatment or diagnosis
- Health insurance information, including insurance policy number or subscriber identification number, claims history or appeal records
The total number of impacted consumers has not been release, but includes at least 2,206 Montana residents and 11,088 in Washington. The data breach was disclosed to the Montana and Washington Attorney Generals' offices on Aug. 12, 2025.
